To set up a New Account, please fill out the below "Contact Us" form (or email us requesting paperwork with your preferred method of payment [Credit Card or Purchase Order]). Once we receive your request, we will reach out with further instructions and the required forms to be completed, which may include:

• New Account Form
• Standard Rental Agreement
• Credit Card Authorization

All required forms must be completed and submitted prior to commencement of Rental Services.

Please note: All productions must request and complete paperwork for each new job.
